One email to privacy@premierprivategp.co.uk covers all the personal data we hold on you within the patient portal app.
Your account credentials, login records and access rights are permanently removed from all our systems.
Name, email address, phone number and all other personal contact information stored in the portal.
Settings, notification choices and any account-linked configurations you have set up in the app.
All personal data not subject to a legal retention obligation is permanently and irreversibly deleted.
We email confirmation once deletion is complete — no chasing required on your part.

Set the subject to "Account Deletion Request" so our privacy team identifies and routes it immediately.
Add your full name and date of birth so we can verify your identity before taking any action.
